Notice: Availability of Form 1095-C Upon Request

Under the recently enacted Paperwork Burden Reduction Act (PBRA), employers now have the option to provide Form 1095-C to full-time employees only upon request, rather than automatically delivering it.

If you would like to request a copy of your Form 1095-C for the 2024 tax year, please contact us using the information below. All requests will be fulfilled within 30 days of the request date.
